Milwaukee Water Works

The City of Milwaukee's water utility — Milwaukee Water Works — accepts ASSE 5110 test reports through a dedicated email address with a PDF attached and the assembly's MWW account number in the subject line. The Cross-Connection Control Section acknowledges receipt within one to three business days; an acknowledgment is the customer's proof of compliance. We do not consider an account closed for the year until the acknowledgment is in our records.

MWW uses Form 295-1C for routine annual tests and Form 295-1D for fail / re-test pairs. Both are PDFs we pre-fill on the iPad in the truck. The fields MWW requires beyond the ASSE form: the assembly's MWW account number, the meter number it sits behind, the building's water-service permit number, the protected hazard classification, and the date of the prior test. We track all of these in our account record.

The 18 suburbs

Outside the city, each suburb has its own water utility and its own paperwork. Our 2025 books had accounts in eighteen of them. Each has a different form, a different deadline, and a different turnaround:

Why reports get rejected

Our 2025 rejection rate was 1.4% — 41 of 2,950 filings. The reasons, in order:

1. Account number mismatch — a customer's MWW account changed (new owner, parcel split) and the prior account wasn't updated. ~38% of rejections.
2. Hazard classification disagreement — the AHJ believed the cross-connection was high-hazard, the prior tester filed it as moderate, and the new test was filed under the wrong class. ~20%.
3. Wrong form — a suburb-to-suburb transfer (customer moved, kept us, didn't tell us). ~16%.
4. Missing gauge calibration sticker date — operator error on the form. ~12%.
5. Other (illegible signature, blank field, off-cycle date). ~14%.

Each rejection is appealed within two business days. We have not had an appeal denied in 2025; the typical path is a phone call with the AHJ contact and a corrected PDF.

Tracking

Every filing has an internal tracking record. The record advances through states: filed → acknowledged → closed. A filing that has not advanced to acknowledged within the AHJ's published turnaround triggers a follow-up call on day +1. A filing that has not advanced to closed within the annual deadline triggers a hard escalation. Customers receive a monthly account-status email listing every assembly, its current state, and its next due date.

Standalone filing

If your annual test was performed by another tester and you need help filing it to an AHJ — common when a property changes hands or when a contractor working in-state had no Milwaukee-specific experience — we will accept a third-party ASSE 5110 form and file it under your account for a flat $85 per assembly. We will not co-sign the form; the testing tester is responsible for the test. We will only file what was provided, exactly.
